Late/Absence Procedures

From Monday 28th February 2022 we will be operating a new system to report your child’s absence

 

Same day absence notification

NEW Reporting Absence

If your child is absent from school due to illness, you must complete the ‘Same Day Absence Form’. This can be accessed from the school website by clicking the 'Report An Absence' button on the right of the home page or by clicking the link below:

The online form must be completed by 8:50am to ensure the attendance marks are updated. Your support with this enables us to safeguard all students. If the form is not completed, parents/carers may receive an absence message.

Please note that this is the only way of informing school of a ‘Same Day Absence’ you can no longer call the school to report an absence.
Parents/carers should complete the form each day their child is absent.

Should you wish to follow this up you can call the office if you feel this is necessary.

 

Reminder of  Term Time Leave request procedure

Term time leave is not recommended or authorised at our school, except in exceptional circumstances. Permission for term time absence must be obtained from the Headteacher prior to the requested leave date. If you wish to apply for such an absence, please complete an Exceptional Circumstances Pupil Term Time Leave Request Form. This form is available from the school office or the school website under- parents - useful forms.

 

Reporting medical appointments

Parents/carers will need to inform the class teacher of any medical appointments via your child’s school planner or email enquiry@whitehousecommon.bham.sch.uk.

If you child’s absence falls below 90% you may be asked to provide appropriate evidence to support the absence such as:

•Medical appointment card with one appointment entered
 •Letter from a professional such as a hospital Consultant or Psychologist
•Evidence of a consultation with NHS 111
•Medication prescribed by a GP
 •Copy of prescription
•Print screen of medical notes
•Letters concerning hospital appointments
•Evidence of a positive Covid-19 test
